A physician-owned multi-specialty medical group of about 350 clinicians and staff, running several clinics across a metro region under the compliance and privacy obligations of a healthcare provider.
Understanding the challenge

The group kept hearing that AI was going to transform healthcare, and it had the budget to act, but no one could say where to begin. Every vendor pitched a different tool, the physicians worried about patient privacy and clinical risk, and the administrators had a dozen ideas with no way to tell which were worth the effort. "We did not need another demo, we needed to know where AI would actually help us without creating a compliance problem," the group's chief operating officer said. The attempts to just try something had gone nowhere, because they started with software instead of the problem. The group was stuck at the starting line, spending nothing but also gaining nothing, while its administrative load kept climbing.

Our approach

The group did not need a build. It needed clarity on where AI would create the most value before spending a dollar on tools. We ran our AI Strategy engagement on a principle the group shared: value before hype, and strategy before software. We started by understanding the group's workflows, systems, team, and goals, then identified where AI could reduce manual work, speed things up, or improve decisions:

• Use-case discovery across scheduling, patient communication, documentation, prior authorization, and billing, surfacing where the real manual load actually sat.

• Opportunity prioritization that ranked every use case by value and effort, so the group could see what to do first and what to leave alone.

• Tool recommendations matched to the group's actual systems and budget, instead of a generic vendor list.

• A governance and risk framework built for a healthcare provider, so every recommendation accounted for patient privacy and clinical oversight from the start.

• A prioritized roadmap with implementation steps and clear next actions the team could actually execute.

Technical innovation

The core of the work was a prioritization model that scored each opportunity on value and feasibility together, weighing the size of the manual load against the data readiness, integration effort, and compliance risk of automating it. Because we run more than fifty operational AI tools inside our own agency, the tool recommendations came from what actually works in production rather than vendor marketing, and every high-value use case was pressure-tested against the group's privacy and clinical-oversight requirements before it made the roadmap. The result was not a wish list, it was a sequenced plan the group could hand to a vendor or an internal team and start executing.

Outcome

The engagement turned a year of paralysis into a board-approved plan in a matter of weeks. Instead of a dozen competing opinions, the group had one prioritized roadmap: 18 high-value use cases ranked by value and effort, the three quick wins it could start immediately, and a clear list of the ideas worth leaving alone for now, all inside a governance framework the physicians were comfortable with. The first quick win, automating part of the prior-authorization workflow, was scoped with a concrete payback before anyone signed a vendor contract. For the first time the group knew exactly where to invest and, just as important, where not to. "We went from a dozen opinions to one clear plan we actually trust," the chief operating officer said. "We finally know where to start."
